Job description
The LPN - Corporate Central Intake Specialist is responsible for the coordination and the processing of all incoming homecare referrals for Altimate care offices in Ohio. This professional is also responsible for obtaining Clinical information such as discharge orders, history and physical, and initial orders for care.
Responsibilities:
- Coordinates intake calls pertaining to the referral and screens referrals for appropriateness.
- Communicates with Marketing team, Clinical team, Branch Manager and Office Manager as to admission approvals.
- Verifies demographics, primary care physician, Insurance coverage and contact information for all new and returning clients.
- Accurately and timely enters referrals into EMR system for further processing.
- Upon completion of the intake process, communicate with client and or family members to coordinate the start of services.
- Communicate with scheduling staff to initial visit time and date.
Requirements:
- Must have an LPN license (Ohio)
- Current knowledge of state and federal homecare regulations
- Working knowledge of clinical terminology
- Customer service experience/phone etiquette, and computer skills
- Prior experience with Insurance authorization process preferred
- Able to work independently and with integrity
- Strong attention to detail, ability to multi task
- Must possess excellent computer skills, and experience with using an EMR system
