Position Summary
The LifeCircles clinical team delivers primary care, palliative care, and end-of-life care for patients aged 55 and older. This position is perfect for the LPN who wants to work in an outpatient setting and be patient-centered, creative, and collaborative in their approach to care.
The successful candidate will join an experienced team and work closely with RNs, MAs, APPs, and other healthcare professionals in a well-supported, team-based environment.
Primary role responsibilities include:
- Assessing and monitoring acute needs and illnesses in the clinic, participant’s home, and facilities (ALF/SNF)
- Working with RNs and clinical team members to manage participant’s chronic illnesses
- Care coordination with the medical team, interdisciplinary team, and with outside agencies
- Patient and Caregiver Education
Skills and Knowledge
- Must possess excellent communication skills with individuals of all ages, especially the geriatric population.
- Must be comfortable in a self-directed environment and maintain flexibility to meet patient needs.
- Must have excellent organizational, critical thinking, and problem-solving skills.
- Must have strong interpersonal skills and the ability to work harmoniously in a team environment.
- Must be technologically adept and proficient in electronic medical records.
Required Qualifications
- LPN Nursing Diploma or Degree from accredited School of Nursing or University
- Current license to practice as an LPN in the State of Michigan
- BLS certification
- Minimum of 1-year experience with the frail and elderly, long-term care, or hospice population preferred
