Responsibilities
The Manager, Care Management oversees a team of Supervisors, RN Care Coaches, Behavioral Health staff and Care Coordinators responsible for care coordination programs. The Manager, Care Management ensures the assessment and evaluation of members' needs are contributing to the care and wellbeing of established care plans. Decisions are typically related to resources, approach, and tactical operations for projects and initiatives involving assigned market area. Conducts and participates in the interviewing/onboarding of new associates. Requires cross departmental collaboration, and conducts briefings and area meetings; maintains frequent contact with other managers across the department.

Required Qualifications
- Active Registered Nurse (RN) license or Bachelor’s Degree in Social Work (BSW)
- Minimum of 5 years experience working as an RN or Social Worker
- Minimum of 3 years of management experience
- Proficiency in analyzing and interpreting data trends.
- Comprehensive knowledge of all Microsoft Office applications, including Word, Excel and PowerPoint
- Progressive business consulting and/or operational leadership experience
- Ability to visit established local market clinics based on business needs
Preferred Qualifications
- Bachelor's degree or advanced degree in nursing or business health field
- Clinical Social Work License (LCSW) or Licensed Master Social Work (LMSW)
- Previous experience overseeing multiple clinic locations
- Previous experience working in a managed care field
- Experience working in home health/home care settings
- Bilingual English/Spanish
Additional Information
For this job, associates are required to be fully COVID vaccinated or undergo weekly COVID testing and wear a face covering while at work. The weekly testing will need to be done through an approved Humana vendor, and unvaccinated associates should follow all social distancing and masking protocols if they are required to come into a Humana facility or work outside of their home.
