Summary
This position is responsible for identifying social determinants of health barriers, assisting in navigating the community-based network of services, providing benefit education, facilitating communication between the member and their providers, and assisting in member development of a self-management plan by performing the duties below.
Essential Duties
1. Provides assistance to members to ensure timely access to medical, behavioral health, oral health, and community services to ensure all services are provided within HIPAA guidelines.
2. Participates as an integral member of an interdisciplinary team and is knowledgeable about the requirements of the Population Health Model of Care.
3. Develops an individualized care plan to meet the specific needs of the member.
Education & Experience
- Bachelor's degree (B. A.) from four-year college or university; or one to two years related experience and/or training; or equivalent combination of education and experience.
Certificates, Licenses and/or Registrations
- Current BLS certification or willing to become certified within 90 days of hire.
- Mental Health First Aid Certification or willing to become certified within 90 days of hire.
- Traditional Health Worker certification through OHA is preferred.
- Doula Certification preferred.
Language Skills
Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of organization.
Mathematical Skills
Ability to calculate figures and amounts such as discounts, interest, commissions, proportions, percentages, area, circumference, and volume. Ability to apply concepts of basic algebra and geometry.
Computer Skills
Has advanced basic computer job skills including logging on to systems, ability to communicate by email, ability to compose documents, enter database information, create presentations, download forms, and preserve/backup important data.
Reasoning Ability
Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
