Coastal Manor is a 39 bed privately owned facility in the heart of Yarmouth. We are currently seeking an MDS Coordinator. The MDS coordinator participates in the assessment process by completing pertinent information on the Minimum Data Set (MDS). This qualified individual is also responsible for completing and updating individual care plans as well as identifying the needs of the residents and projected outcomes.Individual will be responsible for attending rehab and IDT meetings on a weekly basis. MDS coordinator will work closely with the director of nursing, social worker and nursing staff to assure quality of care for our residents. Must be knowledgable on State and Federal regulations. Long term care experience preferred but not required.
Point click care software knowledge preferred but not required. This is a full time position of 35-40 hours FLEXIBLE hours! Pay rate and benefits are negotiable and are based on qualifications and experience. Experience in MDS is preferred but we are willing to train the right motivated individual.
