League of Human Dignity - Level of Care Specialist- Lincoln

The League of Human Dignity seeks a full-time Services Coordinator to work from our Lincoln Office with adults with disability. Services Coordinators assist people with disability to remain in their home and community by coordinating an array of services that support the individual in becoming and remaining as independent as possible. We are seeking a talented and highly motivated individual who has a sincere desire to assist people.
As a Services Coordinator you will determine the eligibility of applicants for the Nebraska Medicaid Waiver Program for Adults with Disability, conduct a needs assessment, and in conjunction with Consumers develop a plan of services. You would assist to recruit independent in home providers, negotiate rates, authorize, and monitor services. This is not an exhaustive description of job duties. This position may require you to work some hours on the weekend.
In order to be considered for this position, you must have a Baccalaureate degree in relevant area of Human Services, and 2 years full-time case management experience in gerontology, rehabilitation, teaching, community health or related social service fields are required. Services Coordination will be provided in Consumers’ homes, so travel is required. Applicants must also be proficient in the use of computers and various computer software programs. You will be expected to provide your own transportation, and we will reimburse you on a per mile basis. Qualified individuals with disability are strongly encouraged to apply.
