SUMMARY

Manage clinic and community referrals to Mobile Integrated Health (MIH) to determine care needs and schedule the appropriate appointments. The MIH Care Coordinator will often serve as the first line of contact for hospital staff and community partners referring patients to MIH. The MIH Care Coordinator will demonstrate competency in explaining MIH services and triaging referrals for appropriateness. Effective coordination with MIH leadership to escalate clinical and social situations as they arise. Completing referrals to case management and social work to meet the social and clinical needs identified by the MIH clinicians. Assisting patients overcome healthcare system barriers to promote optimal health. This position coordinates the patient experience ensuring timely access and seamless coordination of services throughout the continuum of care for MIH.

QUALIFICATIONS

High School Diploma or GED

Two (2) years college level coursework or equivalent experience.

Two (2) years college level coursework preferred.

Two (2) or more years job related experience including appointment and electronic data management

Three (3) or more years job related experience including appointment and electronic data management preferred.

Basic Life Support (BLS) required.

Medical Assistant Certified required.
