Bear Mountain Healthcare of West Springfield is actively seeking an MMQ Nurse (RN or LPN) for a 24hr position.

Our comprehensive continuum of healthcare services includes Post-Acute Care, Short-Term Rehabilitation, Long-Term and Respite Care. Our facility is staffed with healthcare professionals who attend to every patient’s needs.
Job Duties:

MMQ Nurse provides oversight and review of all documentation related to the MMQ process. Completes and forwards all admission and conversion MMQs as well as Semi-annual submission batches to the Massachusetts Office of LTC. -
Responsible for delivering care to residents/patients utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation; and effectively interacts with residents/patients, family members, and other health team members while maintaining standards of professional nursing to provide the highest level of care and safety to our residents.
REQUIRED CERTIFICATES, LICENSES, ETC. :
Current, unencumbered State License
MINIMUM QUALIFICATIONS:
Must be at least 18 years old.
Ability to meet all health, compliance, and competency requirements.
Ability to read, write, and speak English; and communicate effectively verbally and in writing.
Ability to complete required orientation as directed by the facility.
Has successfully completed state required background check and is eligible to work in the facility.
Computer literacy skills (Microsoft Outlook, Microsoft Office, Internet)
High School Diploma
Current, unencumbered MA nursing license.
CPR certification.
Must be fully vaccinated against Covid-19, per MA regulations.
