Job description
Responsibilities of an RN Case Manager Include:
- Manage 10-20 patients.
- Maintain and document patient records.
- Monitor if changes are made with medication or new doctor's order.
- Documentation per agency policy that is consistent with patient plan of care.
- Universal precaution of patient's environment
- Assessments from head to toe.
- Ensuring all clinical administration is done with accuracy .
Training / Orientation: Training is provided in all clinical administration.
Care Type:
- To care for pediatric and adult patients dependent on G-Tube , Tracheostomy and Ventilator.

- Chelsea's Home Health Care
Hours:
- Monday- Friday 9:00AM-5:00PM
- Part-time
Pay:
- $43.00-$55.00 per hour
- To Open New Cases $250.00 per hour
