Concordia Visiting Nurses is seeking an Oasis Workflow Coordinator for a full-time or part-time position which will report into our Administrative office in Valencia, PA.

POSITION SUMMARY

The Oasis Workflow Coordinator is responsible to provide oversight for quality measures and data to ensure that the information submitted aligns with the OASIS guidance and regulatory requirements by reviewing and monitoring OASIS documentation. In addition, the Oasis Workflow Coordinator is responsible for reviewing the Plan of Care and ensuring it is reflective of the patient’s treatment needs. This position uses communication and the resources available to promote quality, cost-effective outcomes to support the continuum of care to all patients served.

QUALIFICATIONS:
  • Licensed clinical professional with a current valid Pennsylvania license (PT, PTA, RN, LPN)
  • Minimum of two (2) years of current experience in home health with progressive responsibilities.
  • OASIS certification recommended, if not, expected to obtain and maintain OASIS certification within 1 year.
  • Knowledge base of agency policies, federal and state regulations.
  • Interpersonal, communication and organizational skills. Evidence of leadership activities.
  • Proficient and knowledgeable on the home health computer documentation system.
  • Reliable, independent means of transportation. Drivers must have a valid driver’s license and drive a vehicle with insurance limits as established by the state
