Qualifications :
Education: Masters Degree in Mental Health Counseling, Social Work, or Marriage and Family
Experience: A minimum of 1 year experience in the Corporate and/or HealthCare field required
Licensure: Licensed as an LMHC, LCSW, or LMFT in the State of Florida is required

Description

• Provides screenings and assessments of new patients for both Partial Hospitalization and outpatient services, both via telephone and in person.
• Devises, completes, and documents on a regular basis on the initial treatment plan, master treatment plan, integrated diagnostic summary, and treatment plan updates.
• Facilitates group therapy sessions and 1:1 sessions on a daily basis as needed.
• Establishes and maintains effective interpersonal relationships with the patient, work team, and all levels of the organization.
• Follows Patient Safety goals
• completes daily tasks as assigned, completes all clinical documentation from group or individual therapy sessions on the day of service.
• Reports incidents to supervisor and Risk Management department within required timeframe in accordance with medical center policy.
