Summary

The PTA will implement and document therapy after an evaluation from a physical therapist in an outpatient facility located in Kendallville, IN

Education

Must have an Associate’s degree from an accredited PTA program.

Licensure/Certification

Must hold a current license in the state of Indiana by the Health Professions Bureau of Indiana. Must be certified in CPR by American Heart or American Red Cross; or must become certified within the first 90 days of employment. Must maintain certification according to expiration date.

Job Type: Full-time

Pay: $22.24 - $33.35 per hour
