JOB DESCRIPTION SUMMARY

Purpose Home Health is a progressive company looking for a fun, energetic person willing to accept the challenge of brining new patients to our company to help with our growth.

The Patient Advocate is an experienced marketer responsible for company growth. As a Patient Advocate, you will be responsible for developing our Home Health presence within Assisted Living Communities, Supportive Livings (Intellectual and developmental disabilities organizations), Veterans Administration, and Home and Community-based residences.

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

• Participate in short and long-range planning to achieve company goals and objectives.
• Responsible for complete referral process, including identifying home health needs, verification of benefits, and coordination of care.
• Cultivate new and existing relationships resulting in company growth.
• Represent the agency in a professional, prepared, and caring manner.
• Utilizing a CRM platform to record referral and sales activities
• Initiate and expand referral network
• Comply with all aspects of the company's compliance and HIPPA regulations
• Performs other duties as assigned
• Familiarity with multiple Technology platforms including but not limited to Microsoft Office, Zoom, Shared Drives

Education/Experience Requirements:

• Bachelor's Degree Preferred
• One year of home health, marketing, or case management experience or a combination of each.
• Must possess a valid driver's license

Bonus Qualifications:

Must be a full-time employee in good standing to qualify for a bonus program.
