Ward 30 Liverpool University Hospitals NHS Foundation Trust (Aintree site) has a vacancy for a full time Patient Flow assistant.

We are offering the opportunity for suitably experienced individuals to apply for this post and work with our fantastic team within the specialty of general medicine

The role involves working as part of the multi-disciplinary team to maintain and expedite patient flow through the ward with the support of qualified practitioners. The successful candidate will liaise with external agencies and care providers, offer support to the nursing team. The successful candidate would be joining a supportive and enthusiastic team who are committed to providing high standards of patient centred care.

co-ordinate/contribute to the safe & timely hospital discharge in partnership with other multi-disciplinary colleagues

Working at ward level the discharge facilitator will be responsible for the timely upload of appropriate information to the patient review app on behalf of the Discharge Service, take responsibility for the timely updating of the information

Maintain momentum of discharge planning throughout the process; supporting and working in partnership with other members of the MDT;

develop strong working relationships with ward teams, Discharge Lounge , Therapy teams, Discharge co-ordinators pharmacy etc., to support and co-ordinate timely and appropriate assessment and safe discharge of people from acute beds

have a current knowledge of the discharge planning arrangements for all patients on the ward at that time

work independently when out of the office base, Escalate concerns & unresolved issues to Discharge Team leader and Discharge Coordinator as necessary and in a timely manner

Establish & maintain a responsive approach to workload and working patterns working within an unpredictable and demanding process which deals with complex information & situations which could require detailed understanding and feedback

Facilitate discharge processes to ensure daily discharges occur before midday

ensure early identification of patients requiring patient transport and co-ordinate to ensure most effective use of resources

  • To provide proactive and responsive support to the discharge process, in line with the Trust Discharge and Transfer Policy, working from within the Discharge Team and working closely with patients, carers, ward based staff, Primary Care, Community Health and Social Care teams and the voluntary sector.
  • To support the patient and staff at ward level to ensure effective discharge planning processes are followed in a timely manner for the safe discharge of patients from hospital and support ward with SAFER process, and Board round.
  • To ensure timely input of information into records relating to flow and discharge i.e. Medically fit patient tracking lists
  • To hold day-to-day responsibility for the patient flow system and update as required.
  • Undertake appropriate patient assessment/screening, identifying delays in the in-patient and intermediate care journey.
  • To inform the MDT and the Discharge Services about delays and potential delays within patient flow/discharge coordination process.
  • To support the senior practitioners within the discharge service in their role as coordinators of complex, multi-agency discharge.
  • Utilise health and social care information systems which enable the clarification of Social Care Needs, through joint working with Social Services and other partners.
  • To provide a single point of contact on a named ward/wards for patients, families, carers and associated people and co-ordinate/contribute to the safe and timely hospital discharge in partnership with other multi-disciplinary colleagues.
  • Ensure patient discharge information provided and Estimated Dates of Discharge (EDD) are recorded in patient notes in the relevant Electronic Patient Recording system.
  • Ensure that patient and family/carer have clear expectations regarding anticipated length of stay.
  • Working at a ward level the discharge facilitator will be responsible for the timely upload of appropriate information to the patient review app on behalf of the Discharge Service. The facilitators will take responsibility for the timely updating of the information and ensure its availability to the MDT.
  • Maintain momentum of discharge planning throughout the entire process; supporting and working in partnership with other members of the MDT; doctors, Nurses, occupational therapists, physiotherapists and other hospital assessment and discharge team members.
  • Establish and maintain productive integrated working relationships with social service and community healthcare colleagues as required.
  • To review all patients on admission and identify those who will require further social care assessments and input/complete the appropriate referral documentation.
  • To develop strong working relationships with ward teams, Discharge Lounge , Therapy teams, Discharge co-ordinators pharmacy etc., to support and co-ordinate timely and appropriate assessment and safe discharge of people from acute beds.
  • The post holder will be expected to have a current knowledge of the discharge planning arrangements for all patients on the ward at that time.
  • To support service developments which improve the discharge process and contribute to whole system patient flow including participation in the SAFER collaborative.
  • To work independently when out of the office base – reporting progress and issues as appropriate. Escalate concerns and unresolved issues to Discharge Team leader and Discharge Coordinator as necessary and in a timely manner.
  • Undertake screening/contact assessments as directed and input referrals into systems
  • Participate in audit, service development and policy development as required.
  • Establish and maintain a responsive approach to workload and working patterns working within an unpredictable and demanding process which deals with complex information and situations which could require detailed understanding and feedback.
  • Undertake to contact by telephone within 24 hours identified patients to ensure a safe and effective discharge
  • To obtain social care information which enable the clarification of Social Care Needs, through joint working with Social Services and other partners.
