We are looking for a Registered Nurse or Licensed Practical Nurse interested in providing pediatric care for a 1-yr old with Spina Bifida and Cornelia DeLang Syndrome. He has a trach, vent, g-tube, O2 and a cardiac monitor. $30/hour for LPN and $45/hour for RN. Nights needed 5 or 7pm - 12 am, Monday - Sunday.

• Provide thoughtful care to clients based on personalized Plan of Care
• Provide in home assessments
• Monitor and track changes in the clients conditions and needs, and keeps the Director of Nursing and any other appropriate personnel informed
• Proactively updates client's family on plan of care, health changes, and concerns

• Current unrestricted RN or LPN license in Utah
• Certified CPR and negative TB skin test or chest x-ray
• Must be able to pass a drug test and a background check
• Licensed driver with valid automobile insurance
• Must be able to stand walk, squat, bend, kneel, reach, twist, push and pull. Must be able to lift 50 pounds.
