What you need:

  • Individuals in this position must have obtained age twenty-one (21) prior to the first day of employment or transfer into the position
  • Current & valid PA licensure as a Licensed Practical Nurse (LPN)
  • Current Cardiopulmonary Resuscitation (CPR) certification and First Aid Training
  • Completion of the Department of Human Services (DHS) Personal Care Administrator course
  • Certification as a PA DHS “Medication Train the Trainer)
  • COVID-19 vaccination or able to obtain an exemption

What you do:

  • Supervises an assigned resident personal care area and assigns work as appropriate. Provides instruction and direction in the methods and techniques of nursing and resident care. Performs nursing duties following licensure scope of practice as needed.
  • Ensures appropriate staffing coverage through the completion and maintenance of schedules. Performs those functions required to manage staff including hiring, problem resolution, evaluations, disciplinary action, and payroll. Assists in developing support plans.
  • Ensures completion of medical evaluations and associated durable medical equipment (DME) forms, pre-admission screening, annual assessments, and support plans (RASP) for all residents in accordance with DHS regulations and the standards of Luthercare. Works directly with family members, outside agencies, and hospital social workers in dealing with residents’ level of care and appropriate placement.
  • Prepares for inspections, including all pre-inspection paperwork/records and completes rounds with inspectors.
  • Coordination of all necessary paperwork for SSI recipients, VA benefits and long-term care insurances.
  • Prepares budget, maintains records to have balanced budget with end of year and provides monthly reconciliation.
  • Provides for monthly in-services and continuing education for staff to ensure appropriate competencies are maintained. Manages training and documentation.
  • Coordinator of team member (new hire and annual) Mantoux testing and Hepatitis vaccines, annual employee flu vaccines, and resident new admission and follow up Mantoux testing as required.
  • Coordinator of admissions, discharges and transfers. Assess residents prior to admission and/or prior to return from hospitalization or another health care provider.
  • Conducts and participates in Town Meetings for campus residents.
