Job description: Advanced Registered Nurse Practitioner or Physician Assistant - Certified

ESSENTIAL JOB DUTIES/RESPONSIBILITIES:

· Functions independently as a primary care practitioner as part of a patient care team.

· Independently assesses acute and non-acute clinical problems.

· Performs and documents physical assessments and patient histories, analyzes trends in patient conditions, develops documents, and implements a patient management plan in response to the data obtained. This also includes assisting in the development of the plan of care in addition to providing appropriate patient/ family/significant other counseling and education.

· Patient management includes the following:

o Ordering and interpreting appropriate laboratory and diagnostic studies,

o Ordering of appropriate medication and treatments,

o Documentation through in-depth progress notes and summaries,

o Completing medication refills, reviewing specialist consult reports, and patient phone consults.

· Collaborates with members of the multidisciplinary team to ensure that patient management strategies are successful in meeting patient care needs.

· Recognizes situations that require immediate attention and initiates life-saving procedures when necessary.

· Enhances a collaborative relationship to maximize the patient’s/family’s ability to make

informed decisions re: goals of care, palliative care, and hospice.

· Provides leadership in the translation of new knowledge into practice.

· Generates knowledge from clinical practice and clinical investigative skills to improve practice and patient outcomes.

· Leads practice inquiry, individually or in partnership with others.

Technology and Information Literacy Competencies

· Translates technical and scientific health information appropriate for various users’ needs.

o Assesses the patient’s and caregiver’s educational needs to provide effective,

personalized health care.

o Coaches the patient and caregiver for positive behavioral change.

· Demonstrates information literacy skills in complex decision making.

· Uses technology systems, with ongoing learning and updates, which capture data on variables for the evaluation of primary care.

EDUCATION AND EXPERIENCE CRITERIA:

· Current licensure in the state of practice is required.

· Certification in Basic Cardiac Life Support required.

· Must have a current DEA number for schedule II-V controlled substances.

Job Types: Full-time, Contract

Salary: $95,000.00 - $110,000.00 per year

Schedule:

• 8 hour shift
