POSITION: HUMAN SERVICES PROFESSIONAL/ASSISTANT DIRECTOR
Job description
REQUIRED QUALIFICATIONS:
· Bachelor’s degree from an accredited college or university in the area of behavioral or social sciences, social work, or psychology, childhood education and (2) years of paid work experience in the field of social services or human service delivery.
· Must be at least 21 years old or older, possess a valid driver’s license and car insurance, and have no criminal history.
- Able to work under pressure and handle responsibilities independent of supervision.
- Excellent communication skills (written and oral); strong organizational skills.
· Must be able to remain calm and respond quickly in emergency and/or crisis situations.
· Must provide appropriate role model behavior for children.
· Physical capability to lift over 20 pounds, and engage in physical activity including various sports, sitting, walking, bending, climbing, kneeling, twisting, reaching, etc. Must pass drug screen.
· Work effectively and cooperatively with staff and administration.
· Use personal skills and interests to facilitate group and individual programs.
· Be open and flexible to change.
· Patience, tact, consistency, self-awareness, directness, and warmth are other essential job functions necessary for an effective Human Services Professional.
REQUIRED TRAINING:
· RBWO Foundations (to be completed within 4 months of hire date)
· CPR
· ESI/Handle with Care
· Mandated Reporter
· Confidentiality and HIPAA (Health Insurance Portability and Accountability Act)
· Reasonable and Prudent Parenting/Youth Rights and Responsibilities
· CSEC (Commercial Sexual Exploitation of Children)
JOB TASKS/RESPONSIBILITES:
- Staff possible admissions with Program Director to make intake determinations.
- Complete admission with youth and Division of Family and Children Services (hereinafter referred to as DFCS), obtaining all pertinent information.
- Enroll youth in school within 2 days of placement (if necessary). Completes school enrollment requirements, including free lunch forms, for all residents at the beginning of each school year.
- Coordinate with Board of Education to arrange transportation for all residents.
- Complete cover sheet within 72 hours of admission.
- Create file for new youth within 5 days of admission.
- Provide an assessment of the youth’s medical and dental health within 7 days of admission and completes 7 day treatment plan, and oversee medications.
- Coordinator with Direct Care Supervisor to ensure all needed appointments are made.
- Complete a 14 day admission assessment for each youth to determine any additional and/or unmet needs.
- Complete a 30-day treatment plan to identify goals and needs of every youth.
- Update the youth’s service plan at a minimum of every six months and incorporate pertinent progress notes and data in the plan to measure attainment of stated goals and needs.
- Work closely with DFCS case manager to ensure that the youth is progressing with their individual service plans.
- Attend school conferences/IEP’s, panel reviews, court, MDT meetings, TRT meetings, and other related meetings as necessary.
- Complete all necessary discharge paperwork and submits to DFCS case manager in accordance with Minimum Standards of Care.
- Update team spreadsheet as needed.
- Complete and reports two purposeful visits per resident each month in accordance with Minimum Standards of Care.
- Rotate on-call schedule.
- Assist with Behavior Program as needed.
- Consult with counselor to ensure all areas of treatment plan are being addressed in the counseling sessions for the youth.
- Complete and uploads all required documentation, including ECEM/EPEM, in the SHINES system within time frames dictated by the Minimum Standards of Care.
- Act as Assistant Director regarding any/all issues with staff or youths in absence of Director.
- Complete all monthly progress notes on each youth’s progress, goals, problems, etc. as required by Minimum Standards of Care and RCC.
- Maintain all youth files and ensures all required documentation is obtained from DFCS in a timely manner.
- Perform all duties within licensing requirements and The Cottage policies and procedures.
- Attend weekly administrative team meetings as directed by Program Director.
- Other duties as assigned by Director.
Job Type: Full-time
Pay: $48,000.00 - $50,000.00 per year
