Position Description: This position is responsible for assisting in the recruitment, education, and provision of technical assistance for Advanced Medical Home Plus (AMH+) and Care Management (Care Management Agencies) Providers in order to meet the Care Management needs of our members. This position is responsible for continuously assessing the capacity needs for Care Management and works in conjunction with the clinical and quality management team to conduct readiness assessments.

Responsibilities and Duties:

• Prepare draft Capacity Building Monthly Report and submit to supervisor for review and submission to the Department on the last day of each month. Staff will use the Department-defined template, with progress updates on achieving milestones.
• Work in collaboration with the clinical and quality management team to create an individualized transformation plan to assist providers as they move through the AMH+ and CMA certification process.
• Review reports provided by the Department of CMA’s and AMH+ providers who begin the certification process and outreach to those providers to provide support and guidance and invite them to our monthly Care Management meetings. Monitor the status as they go through the Desk Review, Site Review and Readiness Review process and provide technical assistance as needed.
• Assist in ensuring network capacity Care Management by assessing the Care Management provider network and identify gaps and needs based upon the Network Access Plan and collaboration with clinical team.
• Collaborate with Clinical Team on members’ capacity needs in order to monitor and ensure that capacity is available through our CM and AMH+ Provider Network.
• Monitor the Care Management Training Plan through our internal Provider Training Platform to ensure that all CMA and AMH+ providers are aware of the required trainings, provide technical assistance and monitor completion of the trainings.
• Assess for additional training needs.
• Access workforce development across the catchment area through surveys, outreach to providers and in scheduled meetings.
• Monitor milestones for each CMA and AMH+ and provide monthly and quarterly reports to supervisor of status.
• Provide technical assistance to providers on the use of NCCARE360 to support the requirement for them to identify community-based resources and connect their members to those resources.
• Maintain a Frequently asked Q&A document for questions about Eastpointe’s Care Management Data System from the provider network. Collaborate with internal resources to ensure that the correct answers are provided to the provider network.

Education/Experience: Master’s degree in psychology, social work, education, health, business administration or related human service field and two years of experience in professional level treatment programming for the applicable client populations; or a bachelor’s degree in one of the fields listed above and three years of experience as indicated above.
