Job Description:
This team leader ensures the individuals’ medical, emotional, and safety needs are met; advocates for individual rights and needs; and facilitates habilitation. The team leader also manages, trains, and develops direct support professionals; works in cooperation in response to inquiries from individuals, families, and others internally and externally; and performs administrative duties. This position carries out its duties and responsibilities with Capitol
Care’s core values.

Education and/or Experience:
High School Diploma/GED and one year experience working with people with Developmental Disabilities; Bachelor's Degree in Human Services preferred.

Qualifications:
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Must be 18 years or older and possess a valid non-probational driver’s license.

Additional Requirements, Certificates, Licenses, Registrations:

-Clearance from the Department of Human Services following a federal and state background check, as well as checked against the Central Registry and Child Abuse Record Information.

-Pre-employment physical; Pre-employment tuberculosis test; Pre-employment and random drug screen as required by The State of New Jersey Department of Human Services.

-Clearance based on a 3-year and annual motor vehicle abstract assessment.

-Must be able to successfully complete required training which includes, but is not limited to:
American Red Cross Adult First Aid & CPR
Crisis Management
Medication Administration with on-site competency assessment
Preventing Abuse and Neglect with on-site competency assessment
Positive Behavior Supports
Employment Specialist Foundations (if applicable)
12 hours of professional development annually per calendar year prorated at one hour per month
Specialized Training as required

Essential Duties:

Essential Duties and Responsibilities including but not limited to the following as well as any other duties as assigned by supervisory staff.

  • Daily monitoring computerized programs including, but not limited to: Dashboard, Therap, and Time America.
  • Assists with overall management of day to day operations of Residential programming.
  • Audits, completes inventory, and delivers cleaning/food supplies as necessary.
  • Ensures quality programming and services through the development and maintenance of appropriate program services.
  • Facilitates program growth and development according to identified needs. Represents and promotes program services within the community through speaking engagements.
  • Assists with admission process for all new referrals to the residential program, including meet and greets, intakes, etc.
  • Assists with all approved outings and documents accordingly.
  • Maintenance of records for all individuals within the program, including progress notes, T-Logs; maintenance of all agency documentation related to program operations to ensure compliance with regulatory bodies.
  • Ability to assess an individual's status including progress towards goals/outcomes as well as identifying areas that need improvement.
  • Assists with individualized service plan meetings, communicate assessment outcomes, review and implement individualized service plan including goals/outcomes, and conduct training with staff.
  • Ensure individuals are safe, healthy, and in a clean environment by conducting daily facility inspection and delegating cleaning and maintenance of the facility.
  • Monitor compliance of medical recommendations including medication supply and associated documentation, annual appointments/associated documentation, prescribed diets, and treatment/behavior plans.
  • Provide assistance in daily activities, which may include bathing, feeding, and toileting.
  • Adhere to all confidentiality policies, practices, and laws.
  • Conduct initial training on routine responsibilities and other essential job duties as required.
  • Demonstrate an understanding of and compliance with regulatory standards.
  • Adhere and enforce all Agency policies and procedures.

Communicates effectively and professionally with peers, leadership, guardians/family, and stakeholders via phone and email.

  • Respond to emergencies by implementing emergency procedures including fire evacuation, and other crisis response procedures.
  • Attend all required meetings.

Physical Demands:
Must be able to address emergency issues with individuals with the average weight between 100-300 lbs. While performing the duties of this job the employee is frequently requires to stand; walk; sit; reach with hands and arms; climb or balance; stoop, kneel, crouch, or crawl and talk or hear. The employee must frequently lift and/or move up to 50 lbs. Specific vision abilities required by this job include close vision, distance vision, peripheral vision, depth perception and ability to adjust focus.
