LVACT Registered Nurse provides the medical and psychiatric assessment and direct care as appropriate to state laws and as prescribed by a licensed physician. These services are designed for individuals diagnosed with a serious and persistent mental illness and have met the criteria for participation in the Lehigh Valley Assertive Community Treatment (ACT) Team. These responsibilities include initial nursing assessment of the medical and psychiatric status of individuals, collaboration with the physician, instruction regarding disease process, medication, symptom management, and activities and interventions to enhance well-being. In addition, the Registered Nurse will assume an active role in the team process designed to maximize collaborative efforts. This is an hourly position with competitive on-call stipends and an $8,000 Sign-On Bonus ($2,000 after 30 days, $2,000 after 90 days, $2,000 after 6 months, $2,000 after one year).

The mission of Resources for Human Development (RHD) is to provide caring, effective and innovative services that empower people of all abilities as they build better lives for themselves, their families, and their communities. RHD’s multi-faceted programs serve people with a variety of challenges that include intellectual and developmental disabilities, mental illness, substance abuse, and homelessness.
Some Essential Functions of the Position:
  • Obtain the consumer's medical history and assess for level of care.
  • In collaboration with the consumer and physician, formulate a service plan designed to promote good health practices and participation in the recovery process.
  • Conduct the review of the service plan with a particular focus on psychiatric and physical health stabilization/management.
  • Communicate with physicians to clarify/mediate.
  • Perform the nursing process (assessment, identification of problems, planning, implementation, and evaluation).
  • Assess signs and symptoms and report pertinent information to the consumer’s FMD or psychiatrist.
  • Coordinate Clozaril reporting to adhere to regulations and requirements.
  • Perform skilled nursing activities as prescribed by the consumer’s physician and within the purview of the law. Interprets and carries out physician orders accurately and promptly.
  • Consult with team leaders and Clinical Coordinator regarding high-risk, problem-prone consumers.

Required Education and Experience:
  • A graduate of a school of professional Nursing - Associate's degree accepted, BSN preferred.
  • A current license to practice in Pennsylvania is required.
  • Certification in Psychiatric Nursing specialty from American Nurses Association preferred.
  • One year of previous working experience in the nursing field.
  • One year of previous work experience in the nursing field and additional experience working within the mental health field are preferred.

About the Program:

Lehigh Valley ACT's recovery-focused, multidisciplinary, assertive community treatment programs provide intensive, individual support for people in Lehigh and Northampton counties who are working to overcome the barriers to recovery from mental illness. Assertive Community Treatment Teams provide: Psychiatric assessment, medication education, and monitoring, treatment for co-occurring disorders, Dialectical Behavioral Therapy, psychosocial services, and education (including Wellness Management and Recovery, SAMSHA evidence-based IMR), and intervention to help strengthen the individual’s networks. Blended Case Management Services affords the collaborative coordination of services and resources supportive of recovery and growth. Individuals work with case managers to increase personal and financial independence, overcome barriers compromising access to treatment, build and expand capabilities in roles and relationships within the community and, acquire skills to support self-determination and self-management of the symptoms or challenges related to their illness. HUD funding allows LVACT to respond to the residential needs of participants who have been diagnosed with serious, persistent mental illnesses and have histories of homelessness.
