Job description

RN in Quality Dept role, both clinical and case management in nature, supporting care for the CKD and ESRD patients of RNA.

Conducts phone triage and collaborates with the providers to properly address patient needs.

Intakes critical lab results and coordinates with Providers to update the plan of care for the patient.

Provides transitional care coordination for post-discharge patients.

Participates in anemia management, modality and transplant navigation, and chronic care management.

Provides education on kidney disease, diet, medication, and treatment options. May be on phone, in-person 1:1 or class settings.

Serves as resource for Medical Assistant staff.

Makes arrangements for dialysis initiation.

Supports prior authorizations, medication refills, notification of test results, etc. to suppor the plan of care for the patients.

Job Type: Full-time

Pay: $38.00 - $43.00 per hour

• Associate (Preferred)

Experience:

• Nursing: 5 years (Preferred)

License/Certification:

• BLS Certification (Required)
• RN (Required)

• 25% (Preferred)
