Responsibilities:

Collaborate with other members of the healthcare team to manage care for assigned patients
Ensure the provision of outcome-oriented patient care
Assess discharge needs and available resources
Maintain communication among healthcare team members and family
Coordinate services along a continuum, including pre and post hospital needs
Meet all documentation requirements

Requirements:

BSN degree
Current Massachusetts RN license
Minimum of 3 years prior hospital Case Management and Utilization Review experience required
Full COVID-19 vaccination + 1 booster required to work at our facility
