The RN will work to provide the highest quality of care to patients utilizing the Plan of Care provided to ensure patient's needs are met in relation to safety, well-being and satisfaction. Bilingual Spanish speaking is a HUGE plus but not required. WellSky/Kinnser & Home Health experience is a plus.

JOB RESPONSIBILITIES

• Provides safe and effective care for the client based on individualized Plan of Care.
• Provides prompt communication of changes in patient medical status/emergencies to Team resulting in effective management of cases.
• Provides safe and effective nursing for the patient, based on the nursing scope of practice and individualized Plan of Care in conjunction with Nurse Supervisor/Manager of Clinical Practice. May include, but not limited to, medication administration, wound care, infusion, transferring & positioning patient, catheter care, effective communication with the client, family, and Team Case Manager
• Completes Starts of Care, using OASIS Assessment tool and performs supervisory visits.
• Demonstrate empathy, caring and self-confidence; ability to pay attention to details resulting in thorough follow through; anticipate problems in advance and knows whether to diffuse or escalate; prioritize and demonstrate a sense of urgency.

REQUIRED EXPERIENCE & EDUCATION

• Current RN license, state of Florida
• Excellent communication skills; team player; interpersonal skills; good collaboration skills; customer service skills; ability to use the electronic health record for documentation and communication regarding client care.
• Must have wound care and infusion skills
• 1 year of Home Health Medicare experience preferred
• current level 2 background on the AHCA website
