West Valley Territory: Peoria, Glendale, Scottsdale, Surprise, Tolleson, Litchfield Park, Avondale, Anthem, Sun City, Sun City West, Goodyear, and Buckeye

Responsibilities:
- Provide thoughtful and quality home care. Provide full nursing care including but not limited to; IV (PIC, TAC, PORT PICC). wound care, medication management
- Monitor and track changes in the client/patient’s conditions and needs.
- Document and update the Director of Nursing
- Proactively updates client/patients’ family on plan of care, health changes, and concerns
- Participate in the On-Call Rotation
Requirements:
- Required to have a minimum one (1) year of documented experience in a home health agency
- Current unrestricted RN in Arizona (Prefer, Bachelor’s Degree in Nursing, not required)
- Current Negative TB test
- Ability to secure a Fingerprint Clearance Card
