ChristianaCare HomeHealth is seeking a Full Time 40 hours a week RN Liaison. As a Nurse Liaison for ChristianaCare HomeHealth you will identify home care needs of patients to ensure a smooth transition from acute care and post acute care settings. The Nurse Liaison supports the intake process by interacting with discharge planners, case managers and patients within the acute and post acute care settings.
Requirements:
- RN must either possess a BSN or be willing to acquire within three years of start date
- 1 year experience in home health, hospice or care coordination preferred
- Ability to work and travel to post acute care settings
- Must commit to work 1 weekend a month, or less, and 2 holidays a year
