Steps 2Wards Success, Inc. is currently seeking a Lead Rehabilitation Coordinator to provide PRP services to youth in Baltimore, MD. PRP experience and experience with severe and persistent mental health disorders preferred.

Psychiatric Rehabilitation Services (PRP)are therapeutic supportive services provided in the community by a well-trained Rehabilitation Coordinator, for youth and adults who are active participants in mental health treatment/therapy. It is the goal of PRP services to maximize the community integration of our members and enable them to be successful and satisfied in the environment of their choice by working together to develop useful rehabilitation goals.

Duties Include:

· Rehabilitation Counselor will support participants by following a treatment plan and implementing treatment goals developed by a licensed social worker.

· Collaborating with parent’s therapists and treatment team in order to address mental health needs and behavioral concerns.

· Completing documentation for each day of service provided using electronic medical records. Participate in Supervision monthly to consult on current case load. Participate in once a month mandated training.

· Document each encounter had with each participant utilizing the format provided by the agency in accordance with all laws and regulation of COMAR, CARF Regulations and Optum Maryland.

· Align supportive services that will promote the health of each participant in consideration of the holistic approach designed by Steps 2Wards Success.

· Attend agency community events created to educate, empower, build healthy relationships with families and expand the brand of Steps 2Wards Success throughout the community.

· Other duties that may arise from time to time that nay be assigned to the employee.

Requirements:

-PRP experience preferred/child welfare experience or case management experience necessary

-Comfortable with and Experienced using computer software (Google/ Microsoft Word)

-Experienced in the mental health or human services field

- Able to manage a flexible but committed schedule

- Able to pass a criminal background check.

Required experience:

• A minimum of 2 years of experience with case management
• 2 years of human service/behavioral health/mental health experience

• Active listening skills: Excellent listening skills will allow you to understand what your clients are sharing with you and subsequently provide the help they need.
• Verbal Communication: Your clients must be able to comprehend what you're telling them so they can take recommended actions.
• Interpersonal Skills: You must be able to understand your clients' reactions and be able to persuade them to make necessary changes.
• Service Orientation: You should have a strong desire to help others.
• Critical Thinking: You must be able to identify and then evaluate possible solutions.

Pay: $24.00 - $27.00 per hour

• Weekend availability
