POSITION DESCRIPTION:
The Resident Care Coordinator (RCC) plays an integral leadership role in assisting the Community Administrator in keeping Vanda Health’s model running smoothly. The RCC and Administrator work as a team to manage resident care, oversee medication management, and respond with professional correspondence and documentation to medical providers, outside agencies, family members, POAs and state agencies. Additionally, the RCC assists with the training, onboarding, and career development of Vanda Health team members; manages staffing and related schedules; and works with HR to hire, coach, and promote talent. RCCs must have strong interpersonal and team-building skills and effective communication. The person in this role must be an organized and detailed self-starter with excellent leadership and coaching skills. KEY RESPONSIBILITIES:
Training, Onboarding, and Career Development
- Train new Personal Assistants (PA) and Med Techs (MT) on caregiving, med administration and passing, campus software and computer programs, and Vanda Health procedures and practices
- Train new PAs and MTs as competent and caring caregivers to Vanda Health standards
- Ensure new staff competency through formalized checklists and personal evaluations.
- Ensure training checklists are completed, signed off, and returned to HR before employee works the floor or passes meds
- Oversee ongoing training for all staff to maintain compliance with required annual training (12 hours), including but not limited to dementia training, residents’ rights, food handlers, first aid, areas of improvement/concern needed, and/or overall skill set
- Work with HR, Administrator, and Nurse to ensure proper topics are covered; Conduct in-service training as needed
- Mentor and coach staff for success.
- Monitor talent potential of staff and plan and prepare qualified individuals for opportunities for advancement.
Manage Schedules and Staffing Levels
- Set work schedules for staff, posting at least 30 days in advance (preferably 60)
- Arrange staffing to best cover the floor based on residents’ needs; rearrange as necessary and communicate changes and updates to all parties
- Take call-outs, be aware of upcoming scheduled time off and overtime, and create a plan to minimize potential burn out of staff due to working too many hours and days
- Work the floor and fill in as PA or MT as needed
- Help cover other areas as needed such as assisting in the kitchen, activities, and/or housekeeping
- Be on-call 24/7 on scheduled work days
- On call is defined as having your EC phone on your person, charged and ready to take calls at all times when on duty and being ready to come in to fill shifts as last resort
Administration, Communication, Compliance, and Documentation
- Keep up-to-date knowledge of OARs and know how to research, interpret, and apply the rules to working situations
- Understand how OARs affect your residents, job, training needs, deficiencies, and corrective actions
- Be familiar with and maintain positive relationships with your state agency representatives (DHS, APS, and Ombudsman) and other outside agencies
- Respond timely and professionally to inquiries, requests for information, and meetings.
- Work alongside the Administrator and Nurse to create and write service plans and assessments
- Help organize, participate in, and when necessary, conduct service plan meetings with family members, residents, and/or POAs
- Respond to inquiries, concerns, complaints, and/or suggestions from family members, residents, POAs and hold meetings with stakeholders when necessary
- Audit medication rooms weekly to ensure order and compliance
- Communicate any medication administration issues to Administrator and retrain MT staff as needed
- Communicate with doctors and other outside providers, pharmacies, facilities, and referral agencies to manage medication, admission and re-admits, and appointments
- Be ready for state survey at any time, ensuring compliance with all OARs in all areas, including but not limited to: thorough documentation for training, safety, cleanliness, kitchen standards, service plans, charting, medication management/QMAR, building maintenance and code, fire alarms and safety
- Participate in the state survey process, speak with surveyors, answer questions, and share requested information
- Keep up on all paperwork, documentation, filing, purging of files, and other miscellaneous administrative tasks
- Assist with budgets and ensure spending, labor, and overtime are kept within parameters of Vanda Health standards
Leadership, Teamwork, and Cooperation
- Know and thoroughly understand each position (PA, Med Tech, AC, Chef, Nurse) to enhance working relationships, functionality of the house, harmony, residents and resident care, and the overall business model
- Lead by example with a positive attitude, appropriate workplace behavior and language, and a willingness to model and perform what you ask of others
- Build healthy, appropriate, and supportive relationships with staff, residents, family members, and Vanda Health communities and departments
- Work cohesively with all team members
- Mentor and coach staff to do the right thing, encourage behavior change, effective communication, and performance improvement
- Maintain confidentiality both with residents (HIPAA) and employee privacy, discussing and dealing with personal and performance issues with those on a “need to know basis”
- Attend and participate in daily team huddles and pass downs, house staff meetings, all staff meetings, family night, and/or service plan meetings
- Attend and contribute to staff, team, resident committee, and other community meetings and encourage others to participate as appropriate
- Communicate professionally with and build supportive relationships with all members of the team including manager(s), nurse, chef, AC, and care staff
- Represent change, policy revisions, and management decisions in a positive and supportive manner in line with Vanda Health’s goals
- Participate in Vanda Health events and social functions such as family night, BBQs, and resident outings
HR Functions
- Assist in the recruitment and selection of staff
- Work with and seek counsel from HR for proper documentation, serious violations, performance or behavioral concerns, and terminations
- Assist with disciplinary issues as necessary, sitting in on meetings and participating as needed
- Document disciplinary issues and any other issues of concern
- Use appropriate Vanda Health documents and follow set Vanda Health procedures to ensure consistency
- Be knowledgeable of Company’s policies, procedures, and practices and keep staff informed and accountable
- Assist with timekeeping and payroll functions
Training, Development, and Compliance
- Keep up-to-date on required training and in-service hours (not less than 12 hours per year or approximately one hour per month) including Food Handlers card, First Aid (CPR if required), dementia training, and other state and company mandated training
- Maintain competency in techniques as outlined in the competency checklist
- Comply with HIPAA Privacy Act by protecting confidential patient information
- Follow Vanda Health procedures and adhere to the guidelines set forth for processes that have been put in place
- Maintain knowledge of and compliance with Residents’ Rights and reports any concern immediately to Administrator
- Disclose unethical, concerning, or inappropriate behavior in the workplace to your manager or a superior
Represent Vanda Health Proudly and Professionally
- Be reliable and follow through on all matters, maintain excellent attendance, be punctual and complete workday/shift
- Conduct self with high amount of tact and sensitivity in difficult and unusual situations
- Bring any concerns about unethical/inappropriate behavior in the workplace to the right person or Manager
- Keep conversations with staff, residents, and other visitors light and appropriate to the workplace
- Follow all Vanda Health policies and stay familiar with the most recent policies and expectations in the workplace
- Always maintain safety and report unsafe conditions to management as soon as possible
- Perform other duties as assigned and offer to assist others when you see the need arise
REQUIREMENTS AND QUALIFICATIONS:
Education and Skills
- Associates degree, continuing, or higher education in healthcare, aging/geriatrics, business administration, or related field or Medical Assistant (Certified preferred) or Certified Med Aide, CNA, or CNA-2
- Three years experience in healthcare setting with minimum one year as Med Tech, RCC, or care staff
- One year of experience supervisory or lead position
- One year of experience managing scheduling/staffing with full understanding of coverage needs
- Knowledge of OARs for an ALF/RCF or related licensure
- Experience and knowledge of State survey process
- Excellent verbal communication, good interpersonal skills, and leadership qualities
- Ability to professionally diplomatically communicate with outside agencies, family members, and other stakeholders
- Excellent written communication with ability to professionally correspond via email and/or letter using proper documentation, grammar, spelling, and punctuation
- Ability to make good sound decisions in stressful situations or when put on the spot
- Ability to work in a fast paced environment and demonstrate agility and flexibility
- Must be available to work a flexible and changing schedule as needed and the possibility of working four 10’s, six days a week, and/or be on-call to answer the phone 24-hours per day
- Must be able to work most major holidays

- Able to turn and move individuals who require assistance
- Able to stay on feet and remain active throughout shift
- Able to bend, lift, twist, stoop, stretch, reach through entire shift
