Univaleo is an employment agency (https://univaleo.net) looking for a candidate who can work as a Case Manager RN at a local hospital in Hollywood, California. We have various openings (ER, ICU, telemetry, med/surg, etc.) in multiple locations (Tustin, Culver City, Hollywood, LA, Norwalk)!

Job Responsibilities/Duties

• Perform clinical assessments and reassessments of acute care inpatients for the purpose of performing utilization review, resource management, and safe discharge planning.
• Reviews medical necessity utilizing medical staff-approved evidence-based decision support criteria to determine appropriate level of care and length of stay.
• Collaborates with patients and caregivers to set goals consistent with physician treatment plans, multidisciplinary team for timely discharge planning assessments and reassessments and documents concurrently in the patient's medical record in compliance with hospital policy, and provides appropriate instructions to discharge care coordinators as needed.
• Participates in patient and family meetings.
• Follows HIPAA compliance.
• Contributes requested data for Utilization Management Committee.

Qualifications

• RN license in CA (BSN preferred)
• 1 year of case management experience required
• Computer literacy required
• Knowledge of CMS, Medicare, Medi-Cal, and Managed Care reimbursement
• Familiar with Joint Commission, CMS, CDPH requirements
• CCM preferred

Sign On Bonus Eligibility: To be eligible for the Sign On Bonus, you must have one or more years of acute care RN hospital experience.

Job Type: Full-time

Pay: $48.00 - $63.00 per hour
