Case Manager, you will be responsible for overseeing and managing the care of our hospice patients, ensuring that they receive the highest quality of care and support. You will work closely with our interdisciplinary team to develop and implement individualized care plans for each patient, ensuring that their physical, emotional, and spiritual needs are met.

Job Details
-
- Current TX RN license in the state of practice
- Minimum of 2 years of experience as a Hospice RN, Patient Care Manager, Registered Nurse, or Charge Nurse
- Bachelor's degree in Nursing preferred
- Strong clinical skills and knowledge of hospice care
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Strong organizational and time management skills
