1. Join our family of professionals at Wickliffe Country Place, a 170-bed skilled nursing facility conveniently located near I-90 and Euclid Avenue and offers multiple specialty programs including on-site dialysis in collaboration with the Centers for Dialysis Care, a high-quality rehabilitation program and a physician-led cardiology program.

Job Summary

The primary functions of the MDS Coordinator will be:
Complete accurate assessments, MDS & care plans as assigned

Initiate care plans and supporting activities as assigned

Create and distribute monthly care plan calendars in a timely fashion

Maintain & update all care plans and assessments as required

Monitor & audit clinical records, ensuring accuracy & timeliness

Inform DON of persistent issues related to non-compliant documentation and protecting the confidentiality of Resident & Facility information at all times

Education and Experience

Licensed RN in the State of Ohio

Prior MDS 3.0 and LTC experience

MDS Coordinator will interact with many people so it is imperative to have constant professionalism and a pleasant disposition
