Overview:
Fairview Range is looking for a Registered Nurse to join our Hibbing Home Care and Hospice team. We consider it an honor to provide care for our patients and their families, and serve communities across the Iron Range. The RN Visit Nurse accepts assignments to evaluate, assess, plan and provide nursing services to homecare patients; maintains close contact with the Case Managers in the implementation of the patient’s plan of care; and delegates appropriate care tasks to LPN's and Home Health Aides (HHA). This position is 0-39 hours per two weeks, 8-hour Day shifts and weekends as needed. This does not include scheduled weekday and weekend on-call rotations.

Responsibilities/Job Description:
  • Performs a Comprehensive Assessment according to established policies (including physical, psychosocial and spiritual data).
  • Assesses family and support systems active in the patient's place of residence.
  • Analyzes assessment data, identifies problems and develops the plan of care in collaboration with the patient, caregivers, and resource personnel.
  • Identifies expected outcomes that are realistic, measurable and attainable.
  • Collaborates with the interdisciplinary team to develop an appropriate discharge plan.
Qualifications:
Required:
  • Graduate of an accredited RN program.
  • Current RN Minnesota licensure required.
  • Current BLS certification or attainment of certification within three months of employment.
  • Current MN driver’s license and insurance.
  • Excellent organizational, communication and documentation skills.
Preferred:
  • One year of acute, long term care, or home care experience
