Program Overview: Early Childhood – Family Connects
Family Connects mission is to increase the capacity of families, caregivers, and professionals to support the developmental, behavioral, early learning, literacy, and overall wellness needs of young children promoting school readiness and responsive relationships. Four primary evidence-based programs coordinated through Family Connects (FC) are typically provided in home and community settings and include awareness, education, consultation, prevention services, and therapeutic intervention supports. The FC program is located at Littler Prevention Campus as one of several teams through North Range Behavioral Health, but the staff covers the entire Weld County area. All the EBP services offered through FC have strong research that indicates children reach their developmental potential and arrive at school ready for success; that families, when they incorporate the education provided, can provide safe environments that are stable and supportive; and finally, that parents and adult caregivers are knowledgeable, responsive and interact in a way that promotes strong relationships with young children.
***$1,000.00 Sign-On Bonus Available***
Salary Range:
$19.13/hr. - $26.75/hr.
Salary Placements will vary and may be above the starting wage based on factors including but not limited to location, experience, education, and licensure. Salary placements generally occur between the starting wage and the midpoint of the salary range.
Employees may be eligible for additional pay based on program, evening, night, weekend, or holiday work. Bilingual pay may be available upon successful passing of language validation testing.
Position Summary:
With a Collaborative, Customer First, and Compassionate approach, Case Managers will:
- Embrace the North Range Behavioral Health Mission Statement - Provide compassionate, comprehensive care for people who face mental health and addiction challenges.
- Collaborate with therapists and other relevant staff to coordinate services, and address client needs and behaviors; prevent deterioration and/or hospitalization through consistency and adequate monitoring as indicated by program scope.
- Refer, coordinate, and assist clients to access a wide variety of services (e.g., vocational, medical, social security, housing, etc.) based on need and client desire; aid clients in application processes and provide supporting documentation as needed.
- Offer support, assistance, consultation, and education to relevant providers, family members/caregivers, and other parties when necessary to support clients and minimize barriers.
- Assist clients in improving their well-being and quality of life by coaching in behavioral skills; educate clients to make informed choices; develop coping skills; help clients to problem solve and negotiate daily tasks.
Minimum Qualifications:
- Bachelor’s degree in a related field with coursework in early childhood development is required.
- Bilingual (Spanish, Somali, Burmese, etc.) strongly preferred.
- Must be able to obtain a PAT National certification as required by evidence-based programs.
- Valid Colorado driver’s license and a driving record that meets agency insurability requirements as set forth for positions requiring transport of customer/program participants.
