A DAY IN THE LIFE OF A HOME CARE/HOSPICE SCHEDULE COORDINATOR:

The schedule coordinator is responsible for scheduling home care visits by direct staff utilizing the current software scheduling system, and for other scheduling related duties. In addition, the home care scheduler is responsible for providing administrative support to the clinical managers.

• Ensures new orders are generated throughout the day for all disciplines. Generates orders at the end of the day in preparation for the following day's schedule.
• Understands visit frequencies and certification periods in order to problem solve and schedule appropriately.
• Ensures all visits are properly labeled to identify type of visit needed, including prn visits.
• Reviews and assigns all open visits to direct care staff using the scheduling systems functionalities.
• Utilizes the scheduling system's functionality to assign direct care staff as case manager. Reports to Clinical Manager inability to assign case manager.
• Ensures visits are canceled appropriately when patients call to cancel, and communicates with appropriate staff member of cancellation.
• Receives notification of direct care staff's unscheduled absences daily. Identifies open visits and reassigns visits using clinical manager support as needed.
• Generates patient schedules at specified timeframes to determine all future open visits that have not been assigned to direct care staff. Assigns open visits to appropriate staff using the systems functionality.
• Generates patient schedules at specified timeframes of all past assigned visits that are open and have not been completed. Able to use Clinical Explorer and the Missed visit report to cancel the visits and creates a note stating reasons why.
• Tracks and produces a variety of statistical data reports, including, but not limited to, open visits, etc. Data is accurate and complete and produced in accordance with designated timelines and/or as requested to assist the Clinical Managers.

QUALIFICATIONS:

• High School diploma or equivalent required. Successful completion of an accredited LNA program preferred.
• Two years of hands-on home care experience required.
• Must have demonstrated computer skills.
• Valid Vermont State Licensed Nursing Assistant license preferred.
• Valid state driver's license required.
