Job description
The Senior Care Navigators serve as key support staff for the Housing Support Team (HoST) Leadership team to ensure that all HoST sites are running effectively. Senior Care Navigators support and lead daily operations and projects.
Shift is Sunday through Wednesday 7am to 5pm
$2,500 sign on bonus
PRIMARY DUTIES:
- Conduct all typical duties for Care Navigators, as well as the following additional duties as needed or assigned:
o Assist with food service and daily operations by placing food or supply orders, picking up food or supply orders, maintaining food and supply inventories
o Serve as the team lead for the daily schedule of duties
o Support with HMIS client tracking and reporting
o Support with other client data tracking needs
o Support with duties related to partner and/or volunteer relations and donations management
o Work in partnership with all HoST entities: Comitis Crisis Center, Colfax Community Network, Aurora Day Resource Center and Aurora Street Outreach
o Additional duties as assigned
SUPERVISION:
- Senior Care Navigators are supervised by the ADRC Program Manager
- All HoST staff are also supervised by the Community Support Manager and MHBHC Director of HoST
- Senior Care Navigators do not supervise other staff, but may lead daily operations or projects at the request of their supervisor
QUALIFICATIONS:
- At least one year of experience working with people experiencing homelessness
- Knowledge of crisis intervention and de-escalation techniques preferred
- Any experience with harm reduction philosophies, trauma-informed care, and low-barrier shelter services, is preferred
- Demonstrated ability to constructively work with persons in crisis situations
- Demonstrated ability to effectively work with people of all backgrounds
- The ability to listen empathetically to clients while maintaining appropriate professional boundaries; good rapport with the client population
- Excellent communication and computer skills
- Detail-oriented, highly organized, and ability to set and meet goals and deadlines
- Self-motivated and self-directed
- Understanding of behavioral health and homelessness services
- Compassion for the clients that HoST serves and a connection to the mission
OTHER REQUIREMENTS:
- High School diploma required, bachelor’s degree in Human Services, Social Work or related field preferred
- Experience with HMIS preferred
- Proof of automobile insurance and clean background check required
- Some travel (within Denver Metro) and occasional weekend work
- Must be able to work a non-typical schedule to accommodate HoST site operational hours
- Valid Colorado driver’s license.
- Ability to Stand, sit and walk in a fast-paced high energy environment
- Ability to pass background check with the Colorado Bureau of Investigation
- Ability to pass background check with the Federal Bureau of Investigation
- Have or obtain First Aid and CPR certification.
COMPENSATION: This is a full-time, salaried position with benefits and paid time off. Salary commensurate with experience
