If this sounds like you, we would like to invite you to apply for our Senior Nurse Assessor role within one of our Responsive Services Teams. This role works alongside colleagues to provide operational leadership and advanced nurse assessment within a healthcare team which focuses on:

• 1 to 10 days of therapy or nursing support to assess, stabilise, treat and safely discharge patients
• Assessment and delivery of short term reablement focused personal care as part of Admission Avoidance and Supported Discharge
• Reablement care including an assessment of ongoing care needs to support the transfer of patients to the most appropriate ongoing care provider

The successful candidate will join an expanding and developing Responsive service to deliver excellent services to the residents of Sussex, working in collaboration with our system partners across health, social and voluntary care.
