We Are Hiring A Social Services Coordinator at Lantern Park Specialty Care!:

PURPOSE OF THIS POSITION
The social worker provides medical social services to Care Initiatives patients/families/caregivers. The social worker closely works with the community resources to meet the patient/family/caregiver needs. The social worker functions according to operational policies and procedures to meet patient/family/caregiver needs while in compliance with federal, state, and local requirements, Care Initiatives Policies and Procedures, and Care Initiatives Clinical Team Procedures.

ESSENTIAL FUNCTIONS1. Completes the comprehensive psychosocial assessment/evaluation of patients and families.
2. Evaluates patient/family/caregiver's emotional, social, financial, and environmental resources.
3. Identifies appropriate psychosocial problems, interventions and goals for care plans.
4. Participates in the development and implementation of initial and comprehensive care plans.
5. Regularly re-evaluates patient and family needs revising care plan as needed and communicating assessments to the interdisciplinary team (IDT).
Qualifications

• Education/Certification/License: Bachelor of Social Work degree from a school of social work accredited by the Council on Social Work Education and must have a current, valid Iowa social work license (LMSW).
• Current Iowa Driver’s license and automobile liability insurance
• Knowledge
• Knowledge of end-of-life care related to social services.

Experience

• Two years of experience in working with terminally ill persons and their families preferred.

Skills/Abilities

• Ability to access businesses and patient homes.
• Ability to establish community relationships, utilize community resources.
• Ability to plan, organize, and effectively communicate both orally and in writing.
