Creekwood Nursing and Rehabilitation is seeking a Social Services Director!
While we prefer a candidate to have experience in long term care, a passion for serving the elderly matters most.
This position assists with assessment and documentation associated with care planning, behavior management and discharge planning. The Social Services Director works closely with residents, loved ones, and community resources in order promote quality of life and person centered care.
Creekwood Nursing and Rehabilitation values its Care Team and is committed to professional growth and clinical excellence.
Social Services degree preferred.
