JOB SUMMARY: The Family Rehousing Stabilization Program (FRSP) is a scattered-site rapid rehousing program for families in the District of Columbia. The Social Worker provides direct management services including identifying and coordinating community resources for families in FRSP to ensure that families are making meaningful progress toward increasing
income and achieving stable housing. The position provides in-home case management and connects families to a variety of individual and/or group supportive services that address both short-term and long-term client needs to sustain housing at the end of the one-year FRSP program.

Hybrid work schedule: 1 day work from office and 4 days in the community or from home.

ESSENTIAL DUTIES and RESPONSIBILITIES:

  • Interview clients for direct service needs, develop a case plan with the client including written short-term and long-term goals and tasks. Make referrals as needed.
  • Conduct ongoing assessment of client needs and reevaluate goals with clients at regular intervals. Deliver and coordinate services to individuals and families and maintain direct contact.
  • Facilitate daily crisis management, acting as a liaison between client and client’s environment.
  • Collaborate with other disciplines including legal, medical, educational, vocational, substance abuse, and mental health professionals, and other service providers.
  • Make appropriate referrals for emotional, legal, medical, dental, education, recreational, vocational, employment, and housing needs.
  • Create and maintain case records, including, but not limited to legal documentation, assessments, case notes, case plans, action steps, service updates, court reports, medical reports, referrals, discharge summaries, visitation records, and educational information in the designated databases such as the Homeless Maintenance Information System (HMIS), QuickBase and STEP within contract requirement timelines.
  • Complete reports and maintain statistical data on an ongoing basis.
  • Engage in continuing education activity to maintain licensure.
  • Demonstrate commitment to improving client service outcomes and customer satisfaction.
  • Perform all other job-related duties as assigned.

EDUCATION and EXPERIENCE:

  • Master’s degree in Social Work.
  • LGSW Social Work license from the appropriate jurisdiction.
  • Two year of experience working with at-risk clients in direct services.
  • Experience working with clients who have a history of homelessness, addiction, and/or mental health issues.
  • Valid local driver’s license and clean driving record required. Use of a personal vehicle
    may be required.

SKILLS and COMPETENCIES:

  • Ability to communicate effectively, both written and oral.
  • Skills in the use of computers, preferably in a PC, Windows-based operating environment.
