Job description

Primary Purpose: Clinical social work assessment, intervention, and consultation to patients, families, interdisciplinary staff, and the community on issues related to Hospice care, anticipatory grief, bereavement, death, and dying. Interventions include counseling, teaching, crisis intervention, discharge planning, referrals to appropriate community resources and other duties as assigned. May be responsible for supervising Social Work Interns.

Reports to the Director of Social Work

Education: Must hold an LMSW (Licensed Master's Social Worker) OR LCSW (Licensed Clinical Social Worker); both from an accredited school.

Experience and Qualifications: 2-years post MSW clinical experience in an oncology, family therapy, child guidance, geriatric, or other relevant social work settings is preferred. Psycho-social and family systems theory related to separation and loss, chronic illness, or bereavement also preferred. Must be eligible for state licensure and agree to actively pursue licensure at earliest possible opportunity. Must be able to observe verbal and non-verbal communications by patients and families.

This is a per diem position with hours available in both the inpatient and Homecare Departments. Hours may be available during the day shift, evenings, and on weekends.
