Job description
#CaretojoinusTSDFT as a Permanent Band 3 Intermediate Care Support Worker?To assist Occupational Therapist, Physiotherapist and Community Nurse working under supervision. To provide rehabilitation to patients within a rehabilitation unit to promote faster recovery from illness, prevent unnecessary acute hospital admissions, support timely discharge and maximise independent living within the recommended time frame of six to eight weeks.
The main duties of this job are to make a difference by helping patients through recovery and returning back to health and independence
The post holder may be required to support community rehabilitation, intermediate care and urgent care response dependant on needs of the service.
The post holder will take part in the Baywide Intermediate Care weekend rota and late duty rota within the zone.

The new team will be multidisciplinary and will have a home first ethos working closely with the GP and community nursing services. The team will be based in a 24 bed rehabilitation unit. This will be a supportive and professionally stimulating environment and an exciting opportunity for the new post holders.
