Position Overview: Targeted Case Managers (TCMs) provide linkage, referral, coordination and monitoring services to clients with severe emotional disabilities (SED) or severe mental illness (SMI) with a need for access to medical, social, educational, housing, vocational or other community service in order to keep them out of a higher level of care. Our full-time TCM’s go above and beyond to provide not only full-time linkage, referral, coordination and monitoring services to a caseload of 20-25 ongoing clients, but they also assist with company events, marketing and community outreach. We want our TCM’s to be an important and integrated part of the communities we serve and an essential part of our daily staff at Alliance. Targeted Case Manager’s are experts in community resources to help keep our clients successful in their community environments and outside of restrictive levels of care such as residential treatment or hospitalization. This position provides face-to-face service contacts in the community, home, office setting and online through telehealth so long as the Kentucky Department of Medicaid Services allows telehealth as a method of service delivery for targeted case management. Schedule:
Monday - Friday: 9am - 5pm in the office or in a community location with clients previously scheduled community location
Monday - Friday: As needed before 10am and after 5pm for team meetings, client contacts or company events
Weekends: Occasionally for company events or urgent client needs

Duties/Responsibilities:
- Must attend DBHDID approved TCM training specific to the assigned targeted population and receive a minimum score of 70 within the first 6 months of employment unless obtained prior to hiring with specific targeted population.
- Attend training annually that provides a certificate of a minimum of 6 CEUs specific to the targeted population being served.
- Attend required monthly group supervision and an additional individual supervision meeting each month.
- Complete prior authorization requests to ensure the targeted case management is a billable service if required by the state of Kentucky.
- Conducts home, community and office visits to assess service coordination needs for clients.
- Conducts and prepares written needs assessments for assigned clients.
- Develops a specific TCM service care plan that addresses the specific needs identified in the needs assessment. The care plans are signed by the TCM, the client, the client’s guardian if applicable, the supervising BHP and any other applicable members of the treatment team.
- Coordinates and arranges for needed services as identified in the treatment plan.
- Assists clients in accessing all needed services within the community.
- Performs follow-up on services to assure they meet client needs.
- Educates clients and their families on the value of treatment.
- Performs advocacy on behalf of clients.
- Suggest and assist with creative ways Alliance can engage with the community - for example, back to school bashes, food and toy drives around the holidays, the now annual trunk-or-treat event, the scarecrow trail, etc.
- Participates in agency continuing quality improvement (CQI) activities.
- Perform ongoing services and contacts ethically and follow all company policies and procedures. Contacts may be performed in the office, school, home, community or online through telehealth so long as telehealth is an allowable method of service delivery in the state of Kentucky.
- Complete crisis plans and discharge plans for each client on the service care plan.
- Be reliable and arrive timely for your appointments and workday.
- Provide crisis intervention for placement and referral needs as required by client needs.
- Present yourself professionally in the office, community and online.
- Establish and maintain rapport with clients, staff and colleagues.
- Complete daily time sheet to reflect contacts provided.
- Communicate and collaborate with the client’s treatment team frequently.
- Maintain contact with outside team members and agencies for solid client care. This may include a medication provider, additional therapist, school, PCP, etc. *Be mindful to always have a Release of Information on file to comply with confidentiality and HIPAA law.
- Maintain confidentiality at all times.
- Attend all required staff or supervision meetings.
- Follow all laws and regulations for licensure and licensure renewal if applicable.
- Complete required documentation of all client contacts on the same date of service. Regulations allow 48 hours, however, with best-practice being 24-hours, that is Alliance’s policy for documentation of TCM contacts and activities.
- Communicate consistently with the Alliance team and reply timely to email, Google Chat, or other messages.
- Update client service plans every quarter and be sure they are signed by all applicable persons.
- Complete additional required annual training(s) such as HIPAA and Medicaid Fraud, Waste and Abuse.

- Assist in creating psychoeducational materials, worksheets for clients, and other creative-type copy assignments.
- Engage in network and marketing with referral sources.
- Upload new resources to the resources chat as they’re found and applicable.
- Attend company wide staff meetings.
- Additional items may be assigned/expected.
Examples of Targeted Case Manager Interventions:
- Assisting with locating and securing housing.
- Linking to employment opportunities.
- Referring to mental health services such as individual, group and/or family therapy, community support, peer support, therapeutic rehabilitation, parenting classes, etc.
- Identifying and assessing client and/or families needs.
- Helping to identify supportive people in a client's life.
- Linking to resources for food, utilities and other basic necessities.
- Helping to navigate social support services such as: SSI, Disability, GED classes, College Applications, Food Stamps, etc.
- Collaborating with team members to ensure the client is receiving the best care possible. The team may include a therapist, community support provider, peer support specialist, primary care physician, psychiatrist, nurse practitioner, social services, the department of juvenile justice, a teacher, or anyone else that may be of support.
- Collaborating with school officials to help address behaviors, peer concerns, IEP issues/implementation.
- Assistance with social activities (camps, sports, 4-H, Boys and Girls Club, church events, etc.)
- Helping to support a client with navigating through needs with their Medicaid MCO
- Identifying and linking with other support services such as the Michele P. Waiver, SCL Waiver and others.
- Monitor quality and access to therapeutic and other needed services.
- Helping with crisis planning for times when life becomes challenging.
- Meets with the client several times monthly to ensure that needs are being met, resources are being utilized, services are going well and continually assessing and adjusting needs and progress.
Documentation Requirements:
- Complete an initial and annual needs assessment. Initial needs assessment must be completed within the first 30 days of service.
- TCM goals, objectives and interventions for the service care plan must be completed as services begin and a minimum of every 3 months thereafter to include having the updated plan signed by the client/guardian.
- All contacts must be documented and signed on the date the contact occurred.
- Contacts need to be spread out over a 30 day period and cannot be back to back unless it is an emergency or services were started toward the end of the month.
- Monthly billing is due on the last day of every month by completing the SED/SMI/BHPH forms in the EHR.
- Case Managers will make a minimum of 5 contacts per month per client.
- Minimum of 2-3 Direct Contacts (30 minutes each)
- Children - 1 Contact with the child and 1 Contact with the guardian
- Adults - 2 Contacts with the client
- Minimum of 2-3 Indirect Contacts (15 minutes each)
- Children - 2 Contacts with child or on behalf of child
- Adults - 2 Contacts with the client or on behalf of the client
- Additional shorter contacts should be kept on a separate log to indicate all the various tasks you completed for or on behalf of the client throughout the month.
Qualifications and Skills:
- 908 KAR 2:260 Section 2 (1) (a) 1.Targeted Case Manager will “possess a bachelor of arts or science degree in a behavioral science”, with “behavioral science” defined in Section 1 (3) as:
(a) Psychology;
(b) Sociology;
(c) Social work;
(d) Family studies;
(e) Human services;
(f) Counseling;
(g) Nursing;
(h) Behavioral analysis;
(i) Public health;
(j) Special education;
(k) Gerontology;
(l) Recreational therapy;
(m) Education;
(n) Occupational therapy;
(o) Physical therapy;
(p) Speech-language pathology;
(q) Rehabilitation counseling; or
(r) Faith-based education.
- Have at least one (1) year of full-time employment experience after completing the educational requirements:
1. Working directly with adults in a human service setting; or
2. Working directly with individuals under the age of twenty-one (21) in a human service setting;
3. A master's degree in a behavioral science may substitute for the one (1) year of full-time employment experience

Benefits:
- Base Salary $20,000 + $180/case/month cases 11-17 billed + $200/case/month for cases 18-25 billed
- (earn $54k/year before bonus incentives with a full-time caseload)
- Quarterly Bonus Incentives of $50 for each month with an eligible caseload of 22+
- 4 weeks PTO (earned and utilized quarterly) - (eligible after 6 months with minimum qualifications met)
