Job Description:
We are seeking a dedicated and experienced PT Home Health professional to join our team at NHC Cookeville/Sparta. The successful candidate will be responsible for providing high-quality physical therapy to patients in their homes and ensuring the best possible patient outcomes.
Responsibilities:
- Provide physical therapy to patients in their homes in accordance with established treatment plans and protocols
- Assess patient's physical abilities and develop appropriate treatment plans
- Educate patients and their families on treatment goals, techniques, and exercises
- Complete all elements of Oasis, including admissions, recerts, rocs, discharges, and evaluations
- Maintain accurate patient records and documentation
- Collaborate with physicians and healthcare team members to ensure the best possible patient care
Requirements:
- Active PT license in the state of Tennessee
- Minimum of 1-2 years of experience in PT home health
- HCHB charting experience required
- BLS certification
- COVID-19 vaccination (medical/religious exemptions accepted)
