Job SummaryAs the Transition of Care Coordinator you will improve the ability of residents to make informed decisions about their care and treatment (i.e. hospice vs. curative care); educate patients and their families about the natural progression of diseases, thereby improving outcome expectations and as a result, improving customer service satisfaction. You will serve as the designated liaison and point-of-contact with hospice in order to oversee compliance with the terms of the facility’s agreements with hospice, and ensure the needs of residents are addressed and met 24/7 through timely and continuous lines of communication.
Education/Experience:
- Registered Nurse with hospice experience required.
- Excellent communication and organizational skills required.
