OACP is the trade organisation for all adult social care providers in Oxfordshire. We work with all providers delivering support and care for any condition in any setting, both registered and un-registered.
We are a membership body, but we have connection and contact with many organisations beyond our membership. Our members are care homes, home care providers, supported living providers, mental health charities and day time support and information & advice organisations.

Responsibilities:
The purpose of this role is to support care providers, discharge teams, patients and their families by carrying out assessments on behalf of care providers.
It is aimed that this will free up resources in providers, facilitate discharges from hospital more quickly and smoothly and reduce post-admission problems and readmissions to hospital.
We are looking for two posts at NHS Band 5.

Benefits:
These posts are initially for a fixed term to the end of March 2023 with a possibility of extension.
Salary: up to £32,934 pa, full-time equivalent, pro-rata to contracted hours.
Time commitment: 37 hours a week, including potential weekend working
Annual leave entitlement for these posts is 26 working days per annum pro rata.
Mileage is paid at HMRC standard rate of 45p per mile.
OACP provides a workplace pension.

Experience:
Essential
Desirable
- Significant experience of working as part of multi-disciplinary and multi-organisational teams.
- Experience of working in social care or health care delivery at a supervisory level or above (with substantial experience for the senior post).
- Substantial experience of working with older peoples’ services.
Desirable
- Experience of care services management.
- Involvement in discharge processes in a variety of settings.
- Understanding of the process of assessing people’s needs from a Hospital setting.
- Understanding of a regulated work environment.
Special Requirements:
Education and Qualification:
Some travel across and outside of Oxfordshire may be required. Travel is paid at the HMRC standard rate of 45p per mile.
- Knowledge of a range of clinical and social care assessment tools
- Minimum NVQ 2 or care certificate with a Level 4 QCF in health and/or social care or equivalent / Level 5 qualification in Leadership and Management / Nursing Qualification/ other relevant experience for the senior post).
- Knowledge of conditions, and illnesses related to the client group.
- Full driving licence and full access to a car for work purposes.
Some travel across and outside of Oxfordshire may be required. Travel is paid at the HMRC standard rate of 45p per mile.
