Daily:
1) Make room and resident rounds. 2) Check 24 hour report for change in condition, concerns 3) Check telephone orders for completeness and transcribe in the computer, 4) Check daily log for completeness of glucometer check, refrigerator temp.in the med room, dining room and resident's room, emergency cart. 5) Check incidents and accident reports for completeness. 6) Check infection report log to ensure that all resident's on antibiotics have infection report. 7) Check nursing documentation on incident reports, antibiotics, change in condition, behaviors, new admissions and readmissions, discharges and any circumstances that warrant documentation and follow up. 8) Check with charge nurses and nurse aides regarding assigned duties. Nurse aides

should not leave without completing ADL's in Matrix. 9) Keep the nurse's station clean, orderly and uncluttered. 10) Check Mars and Tars for holes and accuracy. 11) Check pain flow sheets, behavior monitoring forms.

12) Follow up with nurses regarding lab results.

Weekly:
1) Monitor residents who are on weekly weights, new admissions and readmissions,

change in condition, on tube feeding, dialysis, with wounds, weight loss. 2) Check nurse and medication aide's carts for cleanliness and orderliness. Check for expired meds, insulin's are dated after opening and are within 28 days. In the nurses cart, treatments must be in a different drawer and should not be mixed with other meds. 3) Check weekly skin reports. Any skin issues should have documentation and followed up on appropriately per skin protocol.

4) Follow up on Dietary recommendations by dietician.

Monthly:
1) Check incidents and accidents log, infection report log for completeness. 2) Follow up on pharmacy consultant reports. 3) Follow up on end of the month doctor's orders. A copy should be in the chart while

waiting for doctor's signature. 4) Check on monthly summaries for completeness. 5) Ensure that quarterly assessments are done and completed.
