Summary:

The Vision Screening Quality Coordinator is responsible for successfully meeting our 5 star measure requirements and ensuring successful attention and completion of the Azul Vision’s Quality Programs and standards.

Essential Functions:

• Coordinate and execute mobile screening visits at Primary Care Physician offices.
• Ensure successful completion of Vision Screening core functions including but not limited to: Process fundus imaging through EMR system, perform follow up measures, and report discrepancies and or concerns to direct manager.
• Schedule diabetic patients and update 5 Star program.
• Schedule new and re-directed patients and update internal systems.
• Immediately inform supervisor of 5-star gap list discrepancies.
• Assist & ensure completion of client requests.
• Other duties as assigned.

Requirements/Qualifications

• High school diploma or general education degree (GED).
• Clean driving record, proof of insurance, and current driver’s license required.
• Proof of COVID 19 vaccination and most recent booster is required.
• Position requires ability to relocate to Chino, CA early 2023.
• Saturdays may be required on occasion.
• Adherence to HIPPA and PHI regulations.
• Intermediate MS Excel skills and comfortable with Excel formulas and tools preferred.
• Experience in a health care environment preferred.
• Bilingual (Fluent in English and Spanish) preferred.

Pay: $17.00 - $19.00 per hour
