***SIGN ON BONUS: $1500 for Full-time and $750.00 for Part-Time Position*** (50% at 90 days and other 50% at 180 days)

The Wellness Coach assures that all operational procedures are followed within the Behavioral Health Group Home. The Wellness Coach is responsible for utilizing psychriatric rehabilitation principles to promote recovery, community integration and improved the quality of life for residents through collaboration with consumers, team members, family members, significant others and collateral resources. The Wellness Coach will assist team members and other providers with an understanding of the Psychiatric Rehabilitation Model and the day-to-day practice of this modality. Wellness Coach is responsible for working in accordance with the values and policies of RHD.

  • Hourly Rate: $17.56

FULL-TIME SCHEDULE AVAILABLE: 12AM-8AM MTWTH (mandatory Saturday and Sunday every other weekend)


PART-TIME SCHEDULES AVAILABLE:

Saturday and Sunday 4p-12a (every other weekend)

Saturday and Sunday 12a-8a (every other weekend)

Two Friday's per month 12a-8a

Responsibilities/Duties

  • Utilizing psychiatric rehabilitation principles to promote recovery, full community integration and improved quality of life for residents.
  • To maintain a safe, supportive and recovery-focused environment that fosters growth and rehabilitation as well as facilitates the development of natural supports and self-help skills.
  • Providing compassionate, respectful and trauma-informed care to consumers.
  • Planning creative activities that can be integrated into residential and community-settings.
  • Transporting consumers in company vehicle to appointments and community outings.
  • Preparing health conscious meals and snacks at breakfast, lunch and dinner times.
  • Maintaining a clean, safe and healthy environment by completing a variety of regular household chores.
  • Engaging consumers in a trusting and supportive relationship, providing emotional support within the professional boundaries, as needed.
  • Supporting the individualized recovery plans developed with consumers with the aim of achieving and maintaining physical health, mental health, and emotional well-being as well as participating in the recovery process.
  • Assisting consumers to develop and reach the goals he or she has set in their recovery plan.
  • Practicing and encouraging advocacy to increase independence.
  • Assisting with self-administration of medication and observe for effects. (LLAM training will be provided)
  • Accompanying consumers for scheduled or emergency medical care (if needed)
  • Completing required documentation using a variety of computer-software applications.
  • Supporting residents in activities of daily living, housekeeping, toileting, hygiene as appropriate.
  • Assisting in community Integration Activities and access to community supports including medical appointments, social security access, etc.
  • Completing all other related duties as assigned by supervisor.

Required Experience

  • 1-2 years of residential/long-term care experience preferred.
  • 1-2 years working in an environment that supports those with mental illness or developmental disabilities.

Required Education

  • State of Delaware requires a Bachelor's Degree in Psychology/Human Service or related discipline.

Required Qualifications

  • Bachelor's degree is required
  • Basic computer and internet literacy with the ability to utilize applications like emails, time recording and other programs to enter and retrieve data as necessary to perform the essential functions of the job.
  • Valid Driver's License
  • Must Pass Background
  • Must Pass Drug Screening
  • CPR and/or BLS certified preferred
  • Two-step TB Test required
  • If not Fully vaccinated for COVID-19 you will be required to provide Weekly COVID test results to be work eligible.
  • Must be able to use a computer and provide clinical documentation of services.
  • Medium Physical Requirements: exerting up to 50 pounds of force occasionally, and/or up to 20 pounds of force frequently, and/or up to 10 pounds constantly to move objects.

Program Summary

Grubb Rd Passages is a group home that services women experiencing challenges in their mental health that impedes independence in the community. The home services up to 5 individuals. Currently the home is located in Wilmington, yet we are moving to Newark, DE in the near future. Services provided here focus on safety, skill building, symptoms management, activities of daily living as well as community living to maximize their quality of life and reducing the need for impatient hospitalization.
