Job Title: Admissions Coordinator
Job Summary:
This position will be involved in the day-to-day functioning of the department as they will be working directly with clinical staff in the admissions department. The tasks involved include maintaining census reports, fielding inquiry calls, scheduling appointments, monitoring and managing incoming referrals, completing insurance authorizations, working with patients to complete admissions paperwork, and data entry. This position will report to the A&R Director to improve processes and fulfill daily responsibilities.

In accordance with Colorado state public health order, all healthcare workers must be vaccinated, and the Company must verify vaccine status. Pursuant to this public health order, the following modes constitute as proof of vaccination:
- an electronic entry in the Colorado Immunization Information System (CIIS); OR
- an immunization record from a licensed healthcare provider who has administered a COVID-19 vaccine to an individual who provides services for the facility or agency, specifying the vaccine administered, name and title of the person who administered the vaccine, address of the location where the vaccine was administered, and the date it was administered. A
COVID-19 Vaccination Record Card (issued by the Department of Health and Human Services Centers for Disease Control & Prevention or WHO Yellow Card) which includes name of person vaccinated, type of vaccine provided, and date last dose administered would suffice.
Exemptions
Workers may be exempt from the vaccination requirements by providing a signed exemption form indicating one of the following:
Religious Beliefs- Qualifying Medical Reason
To be eligible for an exemption based on Religious Beliefs, the worker must provide a written statement describing the worker’s sincerely held religious beliefs and how those beliefs conflict with Colorado’s August 21, 2021 Public Health Order requiring certain healthcare workers to be fully vaccinated against COVID-19. The company may request supporting documentation to further evaluate the request for religious accommodation.
- To be eligible for a Qualified Medical Reason exemption, the worker must provide a written statement signed by a physician, physician assistant, advanced practice nurse, or certified nurse midwife licensed in the State of Colorado stating that the COVID-19 vaccination for the employee, direct contractor, or support staff is medically contraindicated as described in the product labeling or authorized by the FDA. This statement should not describe the underlying medical condition or disability but should indicate the probable duration of the inability to receive the vaccine.
A record of signed exemption forms and statements submitted in support of the exemption request must be maintained in a confidential HR Medical file, separate from the Personnel File.
Staff Development Requirements:
Must meet or exceed hospital and regulatory agency training requirements.
Physical Requirements
Position may require walking, standing sitting for prolonged periods of time.
Occasionally lifts up to 10 pounds and carries small objects from waist to chest.
Requires ability to work long periods on the phone to hear and speak clearly over the phone.
Must be able to read and write and specifically record information and read medical records.
Regularly needs to bend, stoop and reach.
Emotional stability necessary to perform the essential functions of the job.
Vision and hearing sufficient to perform the essential functions of the job.
