Position Summary
The LifeCircles clinical team delivers primary care, palliative care, and end-of-life care for patients aged 55 and older. This position is perfect for the healthcare provider who wants to be patient-centered, creative, and collaborative in their approach to care.
The successful candidate will join an experienced Interdisciplinary Team and work closely with other Advanced Practice Providers and healthcare professionals in a well-supported, team-based environment.
Primary role responsibilities include:
- Participating in daily interdisciplinary team meetings
- Completing periodic health assessments
- Assessing and treating acute needs and illness
- Advanced Care Planning
- Patient and Caregiver Education
Skills and Knowledge
- Must possess excellent communication skills with individuals of all ages, especially the geriatric population.
- Must be comfortable in a self-directed environment and maintain flexibility to meet patient needs.
- Must have excellent organizational, critical thinking, and problem-solving skills.
- Must have strong interpersonal skills and the ability to work harmoniously in a team environment.
- Must be technologically adept and proficient in electronic medical records.
Required Qualifications
For NP:- Master's degree in nursing
- Current license and certification to practice as a Nurse Practitioner in the State of Michigan
- For PA:
- Satisfactory completion of a Physician Assistant program
- Current license and certification to practice as a Physician Assistant in the State of Michigan
- BLS certification
- Current DEA registration
- State of Michigan Controlled Substance License
- Minimum of 1-year experience with the frail and elderly or long-term care population preferred
